The Therapeutic Goods Administration (TGA) tracks safety reports in a public database, though it does not release the data in aggregate form. An investigation using automated data extraction revealed that diabetes management devices account for seven of the 10 products with the highest volume of adverse-event reports submitted since 2024. These devices also occupy five of the top 10 positions for reports involving injury or death over the same period.

Technology vs. Regulatory Oversight

The rapid adoption of this technology, which allows patients to track blood glucose levels via smartphones, has introduced unforeseen vulnerabilities. Among the most pressing concerns is the interaction between medical hardware and smartphone software. In some instances, Bluetooth connections to external devices, such as car stereos, have been linked to muted alerts, preventing users from hearing life-saving low-blood-sugar alarms.

While regulators in the United States have issued public warnings regarding the risks of Bluetooth interference, the Australian TGA has not formally passed this information on to domestic users. The regulator maintains that it publishes safety information based on internal analysis of safety signals and trends.

This issue is currently at the center of a major Victorian coronial inquest into a fatal 2023 car crash. A driver, who used an Abbott FreeStyle Libre 2 monitor, reported to the court that he did not hear low-glucose alerts in the lead-up to the accident. Experts noted that while the devices are life-changing, they are susceptible to hardware and connectivity failures that can leave patients without vital information.

Inaccuracy and Clinical Risks

Beyond connectivity, the accuracy of the sensors themselves remains a flashpoint. Many users report significant discrepancies between CGM readings and traditional finger-prick blood glucose tests. When these devices are connected to automatic insulin pumps, such as the Insulet Omnipod 5, inaccuracies can lead the system to administer incorrect insulin dosages, risking severe hyperglycaemia or hypoglycaemia.

The Omnipod 5, which is partially subsidised by the federal government, has been linked to hundreds of adverse-event reports, including cases of diabetic ketoacidosis. Meanwhile, the Dexcom G7 monitor has faced criticism following a US regulatory warning that it relied on unapproved, less-accurate sensor components. While manufacturers emphasize that the benefits of continuous monitoring outweigh the risks and that clinical evidence supports their efficacy, some practitioners remain wary.

Associate Professor Neale Cohen of the Baker Heart and Diabetes Institute has cautioned that regulatory approval for new devices does not guarantee high levels of accuracy. This sentiment is echoed by a coalition of experts who have proposed higher minimum standards for CGM devices, arguing that current benchmarks may be insufficient to protect patients from the risks of automated dosing errors.
