M9 crash kills multiple people after wrong-way motorway collision

Multiple people have died and four others have been hospitalised following a head-on collision on a motorway in County Kildare, according to reporting by Corkbeo. The fatal incident occurred in the early hours of Sunday, 16 August 2026, at approximately 03:00 local time. A vehicle travelling the wrong way down the carriageway struck another car head-on, triggering a massive emergency response and a series of independent inquiries.

According to BBC reporting, the collision took place on the M9 Northbound at Junction 3, near Moone and Athy. Irish Mirror sources indicate that as many as four individuals lost their lives in the tragedy. All of the deceased occupants were reportedly located inside the vehicle that was travelling southbound on the northbound lane. Additional reporting from DublinLive notes that the affected motorway stretch spans between Junction 3 and Junction 4, which remains completely closed in both directions while emergency services manage the site.

Four other individuals sustained serious injuries in the second vehicle involved in the impact. According to official Garda statements reported across the outlets, the injured parties are understood to be a family who were on their way to Dublin Airport when the crash happened. The female driver and front-seat passenger of this second car, both women in their 30s, were removed from the scene to Tallaght University Hospital, where they remain in critical condition. A female rear-seat passenger in her 20s was transported to St. Luke's General Hospital in a serious condition, while a child passenger—understood to be a seven-year-old—was taken to Children's Health Ireland at Crumlin and is also in a serious condition.

The incident has drawn independent scrutiny because authorities were alerted to the wrong-way vehicle prior to the crash occurring. As a result, Corkbeo and BBC report that a senior Garda referred the incident to Fiosrú, the Office of the Police Ombudsman. Fiosrú deployed an on-call team of investigators to the scene to examine whether any actions or omissions by gardaí may have resulted in the death of, or serious harm to, any person. A spokesperson for the Garda watchdog outlined the statutory basis for the probe:

"Fiosrú, Office of the Police Ombudsman received a referral from An Garda Síochána on 16 August, 2026. The referral was made following a Fatal Road Traffic Incident on the M9. One of Fiosrú’s on-call team of investigators attended the scene, and its independent investigation is ongoing."

The watchdog emphasized that the referral does not necessarily mean a garda or gardaí have been accused of wrongdoing. Instead, the statutory investigation is designed to independently ascertain whether or not the acts or omissions of officers contributed to the tragedy. As the matter is under investigation pursuant to section 208(1)(b) of the Policing, Security and Community Safety Act 2024, Fiosrú cannot make any further comment.

This fatal incident follows a separate wrong-way motorway collision that occurred just a week prior on the M50 motorway in Dublin. In that previous event on Sunday, 9 August 2026, gardaí and emergency services were alerted at around 05:30 to a vehicle travelling northbound on the southbound carriageway of the M50. The vehicle drove down the up-ramp at Junction 12 before colliding head-on with another car, sending a total of nine people to hospital, including eight male juveniles who were occupants of the wrong-way vehicle and a man in his 20s who was driving the second car.
